How membership works
Getting established at Symmetry.
Symmetry Primary Care DPC membership is currently open to established patients of Tiffany Pulgiano, DNP. If you are new to Tiffany's practice, our wellness programs are the perfect way to get established, get a head start on your health, and then transition into a DPC membership for ongoing care.

Ongoing DPC care
Once a member, your DPC membership covers all your primary care needs: unlimited visits, direct access, same-week appointments, and a provider who knows your whole story.
Please note: In order for the Direct Primary Care model to work, membership is limited to 200–300 patients. These slots fill up quickly. Book a discovery call to hold your place.

One-time enrollment fee: $75 per person, $200 per family. New DPC members commit to an initial three-month period so we can do meaningful work together. After that, you are free to cancel anytime with 30 days notice. No long-term contracts.

Tiffany brings the knowledge and training to handle far more than most primary care providers. If she can't handle something in-house, she'll find you the most cost-effective path forward.
Direct Access
Reach Tiffany, DNP directly by phone, text, email, and secure messaging. No call centers, no gatekeeping, no waiting rooms between you and your provider.
Unlimited Visits
All new patient and follow-up appointments (in-person and telemedicine) are included. Same-week appointments guaranteed.
Extended Appointments
Every visit is at least 30 minutes. No more rushed 10–15 minute appointments where you leave with more questions than answers.
In-Clinic Lab Testing
Strep, flu, COVID, pregnancy tests, and urinalysis included. Affordable access to additional diagnostics, labs, and imaging through our partner network.
In-Office Procedures
Simple laceration repair, simple skin biopsies, skin tag removals & cryotherapy, EKG, lung spirometry, and Pap smear screening.
Full Scope Primary Care
From chronic disease management and preventive care to hormone health and weight management. No need for a separate specialist for most needs.
FSA / HSA Eligible
Your DPC membership qualifies as an FSA/HSA-eligible expense: a smart way to use pre-tax dollars for your primary care.

Here's what to know before you book your discovery call.
Important: Symmetry Primary Care is a Direct Primary Care clinic, not an insurance plan, and conforms to Utah 31A-4-106.5. Health insurance is still recommended. Insurance plans that work well alongside a DPC membership include casualty insurance and plans with lower monthly premiums and higher deductibles. Your DPC membership covers your primary care. Insurance covers hospitalizations, specialist care, and catastrophic events.
